When a patients has an HLA-compatible donor, if transplantation is chosen, it should be done as soon as possible. However, in older patients at least, previous transfusion history is important - patients who received less than 100 transfusions before transplant having a higher probability of rejection - probably because a poorly suppressed bone marrow is more difficult to eradicate.
