Definition Allogeneic bone marrow transplantation, in which bone marrow comes from a person other than the patient or an identical sibling, is the most common case of marrow transplant. Usually the patient´s sibling or parent serves as the donor, but unrelated donors are sometimes used. (In rare cases, a child may donate marrow for a parent.).The success of allogeneic transplantation depends largely on how closely the HLA antigens of the donor´s marrow match those of the recipient´s marrow, which is determined by special blood tests. (The matching of bone marrow is a much more complicated process than is the matching of blood types. Scientists look at six HLA antigens, and most institutions require a match on at least five of these proteins.) The higher the number of matches, the greater the chance that a patient´s body will accept the graft. A good HLA match also reduces the chance that the transplanted bone marrow will react against the patient´s body. This reaction, called graft-versus-host disease (GVHD), is a potentially serious complication of BMT.
