Definition Cytomegalovirus (CMV) is a member of Betaherpesviridae in the subfamily Herpesviridae. CMV infection is worldwide and usually asymptomatic. CMV may cause a mononucleosis syndrome in the nonimmunocompromised host but is best recognized for causing disease in the immunocompromised host. It may cause disease in almost every organ of the body, resulting in pneumonia, hepatitis, encephalitis, colitis, uveitis, retinitis, neuropathy, and CMV syndrome. As with other herpesviruses, CMV establishes a latent infection of the host. CMV may reactivate during a period of immunosuppression secondary to drugs or intercurrent infection (eg, HIV). Genetic differences exist among viruses, which result in multiple strains of CMV. Differences in genotypes may be associated with differences in virulence. Becoming infected with more than one strain of CMV is possible and has been observed in organ transplant patients. The clinical significance of dual infection is not known. Duel infection has also been suggested to occur in CMV seropositive pregnant women with subsequent intrauterine infection and symptomatic congenital CMV.
