Definition Busulfan is a bifunctional alkylating agent known chemically as 1,4-butanediol, dimethanesulfonate. It is a potent cytotoxic drug that causes profound myelosuppression at the recommended dosage. It is a fundamental part of myeloablative regimens used for patients undergoing bone marrow transplantation for malignant and non-malignant conditions (thalassemias). High-dose busulfan in combination with cyclophosphamide is particularly attractive in children, as significant total body irradiation-related mortality and morbidity, especially involving the developing central nervous system, endocrine system and skeletal system, can be avoided.
