Definition A small number of centers have begun to perform intrauterine bone marrow transplantation, with either fetal liver or bone marrow-derived stem cells as the source of hematopoietic stem cells. Early attempts to perform these procedures were fraught by obstetrical complications and a high incidence of fetal death. Subsequently, improvements in obstetrical technique have allowed adequate delivery of stem cells to the fetal peritoneum. However, there is only one report of such a procedure performed successfully for b-thalassemia, performed at 12 weeks gestation. The assessment of engraftment was based on 0.9% HbA at birth and 30% HbA at 1 year of age. Subsequently, this child has reverted to transfusion-requiring status and has lost his graft. At present, sustained, successful intrauterine transplantations have been reportedly performed only for congenital immunodeficiency syndromes, such as bare lymphocyte syndrome. Such procedures have not yet been performed successfully for thalassemia.
