Definition Hypogonadism causes osteopenia and osteoporosis, and there are therefore medical indications for its treatment. Furthermore, there are significant psychological consequences associated with reproductive endocrine dysfunction. Treatment of gonadal dysfunction has become an integral part of the supportive care of the patient who has reached adolescence. Lastly, with the increasing numbers of thalassemia major patients who reach adulthood, attention to reproductive potential has also increased. Serial studies during the 1980s showed that, with the passage of time, a greater percent of both male and female thalassemia patients who were examined bad spontaneous pubertal development. In females, pubertal development is easily assessed clinically according to the onset of menses and its duration. Studies have shown that the percentage of girls who reach menarche is increasing, but that there is a parallel increase in secondary amenorrhea. There is an inverse correlation between the a parallel increase in secondary amenorrhea. There is an inverse correlation between the age at menarche and the length of menstrual activity, probably reflecting the severity of underlying endocrine dysfunction, which in turn is proportional to the degree of iron overload.
