Definition Hyperactivity of the erythropoietic tissue in the bone marrow, a typical condition in thalassemia major and, to a lesser degree, also in simple thalassemic heterozygosis. In fact, in healthy carriers there is hyperplasia of the bone marrow which only affects the erythroid series, and which is characterized by a high prevalence of normo-erythroblasts.

Context The pre-transfusion Hb should be high enough to inhibit marrow hyperactivity. In general, an Hb level of 9.5 - 10 g/dL is sufficient, though the exact level can vary with the Thalassaemia mutations involved, the patient´s age, and presence or absence of the spleen.
