We report here a case of a true bilineage leukemia in a child associated with arrangement of the MLL gene. The morphologic findings showed two distinct blast populations, one small and lymphoid appearing, with a minor population showing irregular nuclei and a low N/C ratio. This morphologic dichotomy was reflected in the flow results in which there were two different populations, one of B-cell precursor lymphoid immunophenotype CD19+CD10-CD22+CD15+CD33-CD13-, and the other CD19-CD10-CD22-CD33+CD14+/-CD64+CD13+, consistent with myeloid leukemia with monocytic differentiation.
