Osteopenia develops even in adolescent thalassemic patients, and it correlates strongly with gonadal failure and delayed puberty, which are prevalent in thalassaemia. Jensen and colleagues found a low bone density in patients as young as 12 years. This low peak bone mass will reduce the reserve for use in adulthood. As patients age, several other factors cause excessive bone resorption, which will reduce bone mass. Nutritional deficiencies, endocrine failure (hypothyroidism, hypoparathyroidism, diabetes, hypogonadism), desferrioxamine toxicity, persistent bone marrow expansion, iron-induced bone injury, and inadequate exercise are the main contributing factors. Genetic factors also have an important influence on bone mass.
